Dragon's Dogma 2 Dragonsplague — symptoms, cure, and 2026 changes
Recognize an infected Pawn, use Dragonsbaulk Draught, understand Casual Mode and prevent a Normal or Hard Mode calamity before resting in town.
Applies to: Base-game Dragonsplague through Title Update 4.0 · no unsupported hidden-stage timer
Quick answer
On the current build, use Dragonsbaulk Draught to cure Dragonsplague; Capcom added the item and tutorial in September 2026. Before resting at an inn or house in Normal or Hard Mode, inspect every Pawn for pulsing red eyes, changed speech, sickness animations and ignored commands. Casual Mode is different: Capcom says an afflicted Pawn recovers without a devastating calamity even at the terminal stage. No official exact stage timer is published in the reviewed notes.

Current answer: Capcom added Dragonsbaulk Draught in September 2026 as a direct Dragonsplague cure. Older guides that only advise dismissing hired Pawns or sending a main Pawn into the Brine predate that item. The official notes reviewed here do not give an exact hidden-stage timer.
Check Pawns before an inn or house rest
Pulsing or glowing red irises and dragon-like pupils are the most visible reported sign. Capcom made signs such as glowing eyes more noticeable in April 2024.
Game8 reports unusually aggressive or confident lines and ignored commands. Compare behavior with the Pawn’s normal inclination.
Game8 reports headache, coughing, sickness or unsteady animations. Watch the party while standing still.
Capcom’s June 2026 update made Dragonsplague stat boosts appear on the status screen, providing an additional current-build clue.

Use the current cure first
- Stop before resting in a populated settlement.
- Inspect every Pawn and the current status information.
- Use Dragonsbaulk Draught on the afflicted Pawn. Capcom states that it cures Dragonsplague and added related dialogue and a tutorial.
- Recheck the Pawn before taking the inn or house rest.
The September 2, 2026 official note establishes the cure, but the reviewed page does not state where every Draught can be obtained. This guide therefore does not invent a vendor or guaranteed farm.
Legacy fallback: Game8’s pre-Draught guide reports dismissing an infected hired Pawn, or forfeiting a main Pawn to the Brine and re-summoning them. Treat these as older media-reported workarounds when the current item is unavailable, not as the preferred 2026 cure.
Normal, Hard and Casual Mode differ
Normal / Hard
Published guide behavior ties the devastating town event to resting at an inn or owned home with a terminal infected Pawn. Cure or remove the infection before that rest.
Casual
Capcom’s September 2024 update says an afflicted Pawn recovers without a devastating calamity even when the disease reaches a terminal stage.
Capcom’s reviewed notes do not publish the exact number of stages or a guaranteed hours/days countdown. Diagnose from current symptoms and status rather than waiting for a claimed timer.
Prevent a town calamity
Capcom reduced infection frequency and increased sign visibility in April 2024. In October 2024 it fixed an issue where a calamity could affect regions outside the town in which the player rested. Those fixes reduce ambiguity; they do not justify ignoring symptoms on Normal or Hard Mode.
If the calamity already happened
- Check each of the three current save states before overwriting more progress: Autosave, Interim Save and Last Inn Rest.
- If returning to an earlier valid state is not suitable, assess which NPCs are dead at the morgue or charnel house before spending Wakestones.
- The Sphinx’s final large chest reportedly contains an Eternal Wakestone, which PowerPyx describes as reviving multiple nearby dead NPCs. Its acquisition is a separate one-attempt puzzle route.
PowerPyx Sphinx guide · Checked October 15, 2026 supports the Eternal Wakestone source. This page does not promise that one use restores every NPC, all affinity or every quest state.
What changed after launch
| Date | Official change |
|---|---|
| April 25, 2024 | Reduced infection frequency; made noticeable signs such as glowing eyes more visible. |
| September 17, 2024 | Casual Mode Pawns recover without a devastating calamity, even at terminal stage. |
| October 4, 2024 | Fixed calamity effects extending outside the town where the rest occurred. |
| June 10, 2026 | Displayed Dragonsplague stat boosts on the status screen. |
| September 2, 2026 | Added Dragonsbaulk Draught, cure tutorial and new combat benefits for infected Pawns. |
Capcom says infected Pawns can stun large enemies through leg attacks, temporarily attack small enemies without consuming stamina, increase off-balance susceptibility and accumulate the loss gauge more slowly. These benefits do not remove the need to cure before a risky settlement rest.
